PROGRAM: Clinical Services (Assessment & Evaluation)
POSITION TITLE: Clinical Supervisor, LPHA
REPORTS TO: Director of Clinical Operations
JOB TYPE: Full-Time
RESPONSIBILITIES:
Reviews Initial Evaluations, Comprehensive Assessments/Locus Assessments, and other evidence-based assessment tools.
- Sign off on IM CANS and other assessments within required timeline.
- Confirm client’s appropriateness for services through use of the DSM-V.
- In coordination with the Director of Clinical Operations, provide clinical review and consultation on each client’s initial Comprehensive Assessment, Evaluation, IM CANS, Treatment Plan, MH/LOCUS Assessment for proper determination of levels of care established by assigned Mental Health Counselors.
- Participate in internal chart reviews, preparation for site audits and accreditation reviews. Assist in clinical staff documentation reviews and performs audits of Clinical Charts to ensure adherence to State and Federal Requirements and accreditation reviews.
- Participate on the Interdisciplinary Team for Care Plan Development which is composed of individuals who work together meet the physical, medical, psychosocial, emotional, and therapeutic needs of the agency’s clients.
- Attend weekly Administrative Team Meetings.
- Participate in weekly meetings with assigned counselors individually to conduct case assignment and case consultation for discharge planning and IDT case staffing scheduling.
- Provides direct supervision for QMHP’s completing assessments and evaluations of clients.
- Establishes and maintains a trauma informed culture and philosophy within the program.
- Promotes and maintains good relationships and communication between departments and staff.
- Aids in creating policies and procedures that address clinical quality and risk. Assures facility/program honors the rights of persons served.
- Ensures high quality services are being provided to all persons served.
- Collaborates with other staff in implementation of corporate initiatives, and other services, as needed.
- Provide Supervision when needed to assist the Director of Clinical Operations.
- Review and sign off on Clinical Documentation as LPHA as needed.
- In conjunction with the Director of Clinical Operations, provides input regarding the identification and implementation and evidenced-based assessment tools.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- Must meet criteria for qualification as a LPHA under IL. Administrative Code Rule 132. LPHA preferred.
- Demonstrated knowledge of JACHO accreditation standards.
- Demonstrated knowledge of IL Administrative Rules 132, 140, and 2060.
- High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.
- Demonstrated ability to work collaboratively with other departments within TLOC.
- Ability to make appropriate, informed decisions.
- Ability to multi-task, complete a high volume of tasks and projects with little or no guidance.
- Well-developed organizational and time management skills.
- Effective verbal and written communication skills.
- Professional discretion and adherence to confidentiality.
- Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.
