The Recovery Support Specialist (RSS) is indirectly supervised by the Department Manager and provides care to consumers utilizing The Link & Option Center, Inc. (TLOC) principles of Trauma Informed Care. The RSS serves as a member of an interdisciplinary team that provides recovery oriented in-home and community-based services for adults with severe mental illnesses. The RSS provides comprehensive case management and psychosocial rehabilitation through an outreach-oriented set of service activities. The RSS provides crisis intervention, maintains accurate clinical documentation, and demonstrates TLOC’s principles and values when interacting with clients, families, visitors, physicians, and coworkers.
RESPONSIBILITIES:
· Assures that all consumers receive clinically sound triage/referral and ongoing consumer services for behavioral health needs.
· Participates in the development of the consumer treatment plan.
· Provides mentoring, individual and group support, and teaches self-advocacy and motivation through feedback.
· Provides assertive community outreach and sustained efforts to engage individual clients.
· Provides linkage to mental health services and programs including but not limited to psychiatric services, medication management, and representative payee.
· Evaluate the consumer’s treatment plan through follow-up assessment and recommends adjustments based on clinical findings.
· Identifies available support systems to meet the needs of each individual client.
· Responds to crises situations by providing direction, recommendations, and crises intervention.
· Utilizes peer mentoring and support, Wellness Recovery Action Plan (WRAP) facilitation to motivate clients towards wellness and recovery goals.
· Maintains accurate, concise, current clinical documentation. This includes but is not limited to all phone conversations, requests from the client, changes in the clinical conditions and all consultations with team members.
· Integrates ethical principles, evidence-based practice, and research findings into all areas of practice.
· Establishes and maintains good working relationships with the medical, educational, and the social services community.
· Create Safety Plan as needed.
· Assist individuals transitioning from inpatient psychiatric hospitalization and/or incarceration in adjusting to the community.
Client Education
· Utilizes a holistic approach to determine teaching methods appropriate to the individual client.
· Provides psychoeducation to clients and families regarding their illness, medications, and/or treatment plan to supplement information provided by health care professionals.
· Conducts individual, family, and support group training.
· Evaluate the client’s understanding of information provided and ability to follow through.
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
· Must meet criteria for qualification as an RSA under IL. Administrative Code Rule 132. LPHA preferred. People with lived experience highly preferred.
· High degree of professionalism in dealing with diverse groups of people, including clients, staff, and others.
· Demonstrated ability to work collaboratively with other departments within TLOC.
· Ability to make appropriate, informed decisions.
· Ability to multi-task, complete a high volume of tasks and projects with little or no guidance.
· Well-developed organizational and time management skills.
· Effective verbal and written communication skills.
· Professional discretion and adherence to confidentiality.
· Must be culturally competent, empathetic, and able to interact and develop rapport with a diversified population.
$45K (Annually)
