JOB SUMMARY FOR SOCIAL SERVICES DIRECTOR:
We are currently seeking applicants for the position of Social Services Director. This position champions resident advocacy, customer service and individualized plans of care. The Social Services Director will assist in planning organizing, implementing, and evaluating the overall operation of the facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, and the facility’s established policies and procedures.
MAJOR DUTIES AND RESPONSIBILITIES OF THE SOCIAL SERVICES DIRECTOR:
The Social Services Designee will assist the Administrator in the planning, developing, organizing, implementing, evaluating, and directing of social services programs of this facility.
The Social Services Designee will meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
The Social Service Designee will participate in the admission process, discharge planning, development and implementation of care plans and resident assessments.
- · Admission areas of focus
- Work with Marketing to facilitate a quality admission process for each resident
- Oversee the completion of admission paper work ensuring each resident has a signed contract and advance directive UPON ADMISSION (See Advance Directives Policy for specific processes)
- Communicate to the staff and IDT the preferences of each new admission
- · Discharge Planning areas of focus
- Identify the discharge goal of each resident upon admission
- Maintain consistent communication with the resident/resident representative as to the status of the resident progress toward the goal
- Support the resident and the IDT to ensure resident participation to the completion of rehabilitation. Provide encouragement to residents intending to discharge prior to completion of rehabilitation.
- Oversee the discharge process including, but not limited to: referrals to home health, home medical equipment, senior living destinations; establish transportation plans; frequent status communication to IDT
- Monthly report all discharges to the local Ombudsman as per regulation
The Social Services Designee will encourage residents/responsible parties to participate in the development of their care plan, and invite them to care plan meetings accordingly.
The Social Services Designee will accurately and completely document social service actions and interactions in each resident’s medical record.
The Social Services Designee will assist the Administrator in ensuring that staff members are knowledgeable about Resident’s Rights and encourage staff to maintain and enhance each resident’s dignity in recognition of each resident’s individuality.
The Social Services Designee will engage in Advance Care Planning for assigned residents upon admission, and make sure that any Advance Directives are reviewed with the resident/resident representative on a regular basis. The Social Worker will ensure that staff members are made aware of the resident’s code status and end-of-life wishes and will assist with informing and educating residents and their representatives about health care options and ramifications.
The Social Services Designee will advocate for residents and assist them in assertion of their rights within the facility. The Social Services Designee will assist with investigations of abuse allegations.
The Social Services Designee will ensure that residents who display mental illness, or psychosocial difficulties such as coping with grief and loss, have access to appropriate treatment and resources.
REQUIREMENTS FORTHE SOCIAL SERVICES DIRECTOR:
- A bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology is required.
- Current Social Worker license from the State of Illinois preferred.
- Prior experience in Social Service work in a LTC setting preferred.
- Ability to motivate, encourage residents; knowledge of how to adapt life skills to the cognitive and functional ability of each resident and attempt to meet goals.
- Ability to handle multiple priorities.
- Possess written and verbal skills for effective communication.
- Competent in organizational & time management skills.
- Demonstrates good judgment, problem solving and decision-making skills.
- Knowledge of state and federal resident long-term care regulations in an LTC setting.
- Excellent communication, leadership and mentoring skills
