SIGN-ON BONUS: $7,500

The Subacute Unit Coordinator is responsible for the care and safety of all Residents and/or Patients on the Subacute unit. S/he is responsible for supervising direct care to Residents by licensed professionals, Certified Nursing Assistants, and Rehabilitation Aides.

Job Description
The Subacute Unit Coordinator is responsible for the care and safety of all Residents and/or Patients on the Subacute unit. S/he is responsible for supervising direct care to Residents by licensed professionals, Certified Nursing Assistants, and Rehabilitation Aides.
Responsibilities
- Assures completion of a comprehensive nursing evaluation/assessment of a resident’s physical, emotional and/or mental status upon admission/readmission and when indicated; and for appropriate follow up as indicated by the assessment.
- Attends various facility meetings as invited or required.
- Supervises the team leaders, nursing rehab assistants, Certified Nursing Assistants, nursing assistant trainees, unit clerks (if applicable) and completes probationary and annual evaluations.
- Ensures that the physician in informed of any resident’s condition, abnormal lab results, diagnostic test results, consultant recommendations, etc.
- Notifies physician when visits are due per policy, makes rounds with physicians, reports changes in resident conditions and reminds the physicians of policies and procedures relating to care of the residents.
- Mediates family, visitor and staff concerns.
- Completes quality assurance audits as directed by Director of Nursing.
- Maintains an organized and efficient unit.
- A registered professional nurse currently licensed in New York State who possesses an appropriate level of education, training and experience to fulfill all requirements/duties of the position.
- Minimum 18 years of age
- No findings or convictions of resident abuse, mistreatment and misappropriation of property
- Strong written and verbal communication skills
