LPN Admission Screener will be based in a hospital setting and screening for our long-term care and rehab facilities.
Shift: Part-Time (Thursday - Saturday)
Schedule: Thursday & Friday 8:00 AM - 4:30 PM, Saturday 8:00 AM - 2:00 PM
Job Description:
The McGuire Group has an exciting opportunity for a Part-Time Admissions Screener to join our caring and knowledgeable team supporting our facilities in western New York. The admissions screener is works as a liaison between referral sources; and facilities, making both medical and social assessments, coordinating all admissions and transfers to the facility, working closely with the facility admission coordinator and a team of insurance coordinators and leadership in our admission departments.
Responsibilities:
- Responsible for completing admission assessments and recommending patients for long term or subacute care
- Visit designated hospital assignments daily
- Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in carrying out the admissions process
- Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner
- Additional tasks as assigned
Qualifications:
- Degree in Nursing, Social Work or Physical Therapy preferred
- Experience in Admissions Screening and long-term care is preferred
- Good Understanding of Medicare and other Managed Care insurance helpful
- Must be flexible, professional, and reliable
- Computer skills including keyboarding and PCC
- Strong customer service skills
- Must possess a valid New York State Driver’s License and have transportation available for daily job-related travel
