The Meadows at Ashland,a skilled nursing facility located at 1700 Furnas St Ashland, Ne. provides high quality short-term rehab and long term care to our residents. We are in search for a high energy passionate Social Services Designee.
Customary Duties
Assists the social service director in advocating and assisting the residents.
Assists the resident, responsible party, and supportive others in the adjustment and orientation to the nursing home after admission.
Participates in each resident's total plan of care staffing.
Has a knowledge of the community resources available and use them to benefit and improve the quality of care as applicable for each resident.
Takes advantage of opportunities to inform and make community aware of the nursing home's need and value to the community.
Has an open door policy to all residents, family members, and staff to listen to their problems or needs; do effective counseling or referrals to meet these needs.
Assists other departments within the facility when requested, and when it will benefit the residents and/or help the facility operate smoothly.
Plans and organizes family council meetings.
Job Requirements
Experience in long term care.
