Purpose:
The Wellness Coordinator (Clerk) is the administrative arm of the Wellness department. He/she reports directly to the Wellness Director.
Qualifications
Education and Experience:
- Current State Nurse’s Aide registration.
- Experience and passion in working with seniors.
- Able to perform tasks which may be physically demanding such as pushing, bending, and lifting up to 60 pounds.
- Able to maintain positive working relationships with Residents, their families, peers, and other staff members.
- In good physical and emotional health and free of communicable diseases
Responsibilities
Essential Functions:
- Resident Service Plan: Tracks care plans for current Residents and ensures revisions of care plans occur at prescribed intervals. Reminds clinical team of changes in condition that warrant revisions of care plans and confirms that new plans have been prepared for incoming Residents.
- Medical Paperwork: Files and tracks all medical test results and pending orders on the Resident Service Plan charts.
- Medicine: Receives prescriptions from on-duty nurse(s) and submits to pharmacy and arranges to pick up if no delivery service is available. Maintains files on all old prescriptions and pharmacy invoices.
- Insurance: Verifies all active insurance policies for Resident and places orders for any covered and prescribed medical equipment/supplies.
- Preventative Maintenance Log: Updates all medical changes to assure Resident Associates are providing the proper care for Residents.
- Lab Work: Collects and files all physician lab requests and helps to set up appointments.
- Admissions: Collects and reviews all information and insurance cards for newly admitted Residents, checking with Resident Care Director and nursing staff to assure that allergies, attending physician, and other medical information is correct and well documented.
- Medication Administration Records: Receives monthly recaps of all resident MARs from the pharmacy and places on the resident charts for licensed nursing review.
- Durable Medical Equipment: Orders all durable medical equipment ordered by physician. Must verify payment source for equipment before ordering.
- Supply Ordering: Checks on current supplies on an ongoing basis, making sure that all necessary daily supplies are available to meet Resident needs and places orders for any items that are running out.
- Scheduling: Works on setting up a schedule for three daily shifts and advising all department Associates of their schedules.
- Payroll: Checks time punches for all Resident Care Associates for the preceding week and sends report to the Business Operations Manager.
- Resident Appointments: Schedules all necessary medical appointments for Residents, assuring that appropriate referrals have been provided.
- Resident Record Copies: Obtains copies of Resident records requested by attending physicians or attorneys.
- Resident Transportation: Arranges and confirms required transportation for approved Resident appointments.
- Resident Transfer Sheets and PO: Collects and files all relevant resident information chart. This includes resident transfer sheets, and physician orders.
- All Referrals for podiatry and audiology: Schedules all referrals from family and clinical for podiatry and audiology services. Coordinates with visiting onsite professionals the residents needing to be seen.
