Basic Work Schedule is: 8:00am-4:00pm; Monday thru’ Friday.
JOB DESCRIPTION: The Crisis Response Team (CRT) Outreach Specialist provides immediate support and intervention assistance in collaboration with law enforcement for individuals experiencing crises related to mental health, substance use, homelessness, and involvement with the criminal justice system. The Outreach Specialist’s primary role is to promote stability and well-being by helping individuals develop healthy, constructive living patterns while reducing reliance on crisis services and lowering recidivism rates. Operating with a high degree of independence, the Outreach Specialist delivers crisis de-escalation assistance, advocacy, monitoring, linkage to services, and ongoing support to address the complex needs of individuals with mental illness.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Provide short-term crisis intervention assistance, in person, to children and adults in collaboration with CRT Crisis Counselors and law enforcement.
- Conduct scheduled and unscheduled outreaches and follow-ups with clients identified as experiencing frequent mental health crisis, involuntary hospitalizations, and/or criminal behavior history.
- Report crisis situations and collaborate with CRT Crisis Counselors when assessment or stabilization is needed.
- Conduct outreach visits to high-risk individuals, independently or with law enforcement support.
- Provide face-to-face visits with patients discharged from Emergency Departments and inpatient psychiatric units ensuring intensive post discharge follow- up.
- Conduct individual needs assessments to identify and address client concerns.
- Coordinate client access to services by assisting with arranging appointments, coordinating transportation, and ensuring service delivery.
- Connect clients to formal and informal support networks to enhance stability and well-being.
- Assist clients in developing better judgment skills through supportive monitoring and guidance.
- Encourage client participation in appropriate programming and provide assistance as needed.
- Serve as an advocate, facilitator, monitor, and primary resource for assigned clients.
- Build and maintain collaborative relationships with community service providers to improve access to care.
- Work closely with service providers to expedite access to necessary resources.
- Maintain accurate documentation of client/ agency contacts in accordance with program policy and procedure manual.
- Maintain self in professional manner at all times, adhering to Neighborhood Center policy and procedures and in keeping with the Agency’s mission statement.
- Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time and prepared to start working.
- Maintain a valid New York State driver’s license and have access to a reliable vehicle.
- Perform other duties as assigned by program management.
REQUIRED QUALIFICATIONS:
- Experience with and knowledge of the mental health delivery system and community resources.
- Familiarity with the role of the criminal justice system in mental health crisis intervention and care.
- Work experience demonstrating organizational, assessment and communication skills commensurate with this position.
- Experience in securing the cooperation of and working effectively with other professionals and agencies in the community.
- A Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field with a minimum of three (3) years’ experience working in a human services agency.
