What are the responsibilities and job description for the Transitional Coordinator position at The New School of Montpelier?
NSM is approved as an independent school by the Vermont State Board of Education to provide general and special education services to students, ages 5-22 in grades K-12 in the disability categories of Intellectual Disability, Specific Learning Disability, Autism Spectrum Disorder, Other Health Impairment, Emotional Disturbance, Speech or Language Impairment, Developmental Delay, Multiple Disabilities, and Traumatic Brain Injury. Additionally, NSM operates a program for adult alumni with intellectual and developmental disabilities.

Position: NSM is currently seeking a Transition Coordinator that is responsible for working collaboratively with the Program Director(s) incharge of the high school aged population in order to develop a transition program and to provide career and transition services and support to individuals with disabilities, and to special education program staff and employers working with said individuals.
Qualifications: Leadership or managerial experience and experience working with students and/or adults with disabilities. Associate’s Degree or higher preferred.
Other requirements for this position include:
Ensure maintenance of records: Personalized Learning Plans (PLPs), benchmarks, data, life assessments.
Assess work-based skills aligned with Proficiency Based Graduation Requirements (PBGRs).
Facilitate the development and implementation of transition type services based on transition and PLP goals.
Model, train and support Student Support Specialists in working with students on the job and in the community as needed.
Provide individual and small group instruction that align with students goals as necessary.
Coordinate and case manage with Hireability.
Comply with all policies and procedures with the NSM handbook and the Student and Family handbook.
Must have excellent written, verbal, and time management skills. Strong interpersonal skills are required.
Must have the ability to develop, maintain, collaborate positive relationships with all team members.
Valid driver’s license, current vehicle inspection and car insurance is required.
Compensation & Benefits: This is a full-time position, with an annual salary of $40,000 to $44,000.
