Counselor/Case Manager - In-Home Support and Reunification Services (ISRS)
Worksite Location: The Dalles and Hood River (Client’s homes, schools, and communities)
Work Schedule: Full-Time; 40 hours per week; Flexible (based on the needs of clients served)
Salary Range: $54,312 - $57,699 per year
Reports to: Clinical Program Manager

Who we are looking for?
We are looking for someone who is reflective, respectful, intuitive, honest, and self-motivated. We’re looking for someone who is able to connect and build trust quickly with a variety of people. We’re also looking for someone who is detail-oriented, organized, knows how to prioritize tasks, and can work independently as well as part of a team.
What you will do…
- Perform Core Team Member roles and responsibilities as outlined in the Team Member Roles & Responsibilities matrix.
- Provide in-home services and support to families, focusing on safety, and change with the intention of children remaining safe in their own homes.
- Accept referrals from DHS during “normal business hours” and initiate contact within 24 hours of referral acceptance if there is a safety threat.
- Initiate contact with family when needed for reunification.
- Provide supervision and monitoring of children, stress reduction, parenting assistance, accessing respite, social/emotional support, safe housing, accessing basic needs, and connecting to local resources.
- Assist families in communication with DHS and maintain frequent contact with caseworkers.
- Schedule and coordinate services to best facilitate family schedules and travel.
- Complete reports, identifying the current status of the family, barriers, and strategies to overcome barriers.
- Maintain chart notes on each individual or family session.
- Provide monthly progress summaries and closing reports to DHS and stakeholders.
- Keep client files current and up to date.
- Consult with other professionals concerning identified clients or professionals directly involved in providing services for the clients.
- Coordinate with other service providers, schools, and involved individuals and agencies.
- Observe confidentiality, civil rights, and other rights as established by the therapy agreement.
- Make referrals for additional services and or work with clients to obtain necessary services.
- Encourage positive community relations by personal appearance and behavior in the community.
- Other duties as assigned by supervisor.
Education, Experience, and Requirements…
Required:
- Bachelor’s or Master’s degree in Human Services or a related field
- 1 year of experience in behavioral health.
- 1 year of experience/comfort in providing in-home care.
- Valid driver’s license with auto liability insurance and reliable transportation
- Successful completion of a comprehensive background check
