What are the responsibilities and job description for the Case Manager position at The Ole Homeplace?
We are looking for an immediate hire of an energetic, compassionate case manager to coordinate the care of elderly and disabled individuals in the Home and Community Based Waiver Program. The following requirements are needed:
- 1 year Case Management Experience Preferred
- Experience in the Home and Community Based Waiver Program preferred (not required)
- Must display Effective communication and organizational skills
- Multitasker
- 50-75% Travel Required
- Must pass all background checks and drug screening
