POSITION SUMMARY:
The role of the Hospice Chaplain is to develop and enhance spiritual programming that accommodates all residents and patients in need regardless of religion.
POSITION DUTIES/ESSENTIAL FUNCTIONS:
Ordained and in good standing with a recognized faith group, OR
Master's degree in theology, divinity or related field, OR
Completion of clinical pastoral education program, OR
Experience In Hospice Or Palliative Care Preferred
Must meet required background screening including MVR check
The role of the Hospice Chaplain is to develop and enhance spiritual programming that accommodates all residents and patients in need regardless of religion.
POSITION DUTIES/ESSENTIAL FUNCTIONS:
- As an integral part of the Overlook Hospice Interdisciplinary Team, provides spiritual support and guidance to the hospices’ patients and families through assessment, collaboration, and presence.
- Ensures that documentation is complete and timely and that all patient care issues are communicated with the Hospice Director and members of the IDT.
- Provides bereavement support to hospice families via phone calls and in-person visits.
- Participates in hospice memorial services as a member of the Interdisciplinary Team.
- Ensures that patient and families hospice care plans are appropriate and up to date and ensures that physician-ordered services are provided.
- Works in collaboration with Hospice Community Services Coordinator and Hospice team.
