The Pavilion at Queens Rehabilitation & Nursing is committed to improving our patients' quality of life & autonomy through innovative rehabilitative services, and an exceptional level of client-centered care and attention. We are seeking a compassionate and detail focused Social Worker to join our interdisciplinary team of skilled health care professionals at our skilled nursing facility in Flushing, Queens.
Job responsibilities include but are not limited to:
- Serve as a liaison between the care team, residents and their families.
- Provide emotional support to the residents and their families.
- Deliver services to residents in accordance with standard operating procedures, standards of practice, state licensing, and federal agencies.
- Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs, goals to be accomplished for those needs, and the appropriate social worker interventions.
- Develop, maintain and utilize a listing of current community resources that are useful to residents and their families/significant others.
- Ensure or provide support and education to residents and family members to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.
- Coordinate and facilitate residents discharge and care plans.
Qualifications:
- Bachelors degree required, Masters or LMSW preferred.
- Hospital or long-term care experience strongly preferred.
- Caring and helpful demeanor.
- Strong communication, follow-through and documentation skills.
- Resident satisfaction driven.
- Comfortability working with elderly and sick population.
Job Type: Full-time
Pay: $55,000.00 - $70,000.00 per year
