Primary Responsibility:
The Women’s Health Program Coordinator will work to improve the well-being of women by offering both administrative and clinical support to the Women’s Health Provider team and the patients utilizing women’s health services at the People's Health Clinic. This position requires a seamless transition between various roles, including coordinating patient care services as a community health worker and serving as a medical assistant to women’s health providers including the Women's Health Medical Director and Certified Nurse Midwives. Success in the role requires working within a culture of collaboration, excellence, leadership, and respect.
Job Responsibilities include:
- Obstetric Patient Care Coordination:
- Coordinating new-OB patient orientation.
- Scheduling follow-up prenatal visits and ensuring that patients understand their care plans.
- Obtaining up-to-date referral/consult/ultrasound documentation.
- Following-up abnormal laboratory results in coordination with Women’s Health providers.
- Transferring patient to clinical care partner when insurance is obtained.
- Transferring to high-risk management if needed.
- Faxing records to labor and delivery.
- Maintaining Ob spreadsheet (new Ob and EDC).
- IV services.
- Scheduling LiveWell nutrition appointments.
- Connecting patients to community resources when necessary for support of food/housing/transportation insecurity and other social needs.
- Assisting with Patient Assistance Applications for obstetric-related medical management.
- Optimizing Postpartum care per ACOG recommendations:
- Calling patients within 3 weeks of delivery.
- Screening patients for postpartum depression.
- Contraception care coordination.
- Connecting patients with mental health care support when indicated.
- Liaison of lactation/delivery orientation sessions:
- Offering and scheduling patients for sessions and coordinate referrals for appointments when necessary.
- Coordinating patient care with IBCLCs.
- Gynecologic Patient Care Coordination:
- Communicating with patients regarding results.
- Coordinating timely Gyn referrals when communication difficulties exist.
- Cervical Dysplasia program:
- Maintaining/ordering colposcopy/LEEP supplies.
- Maintaining colposcopy list.
- Coordinating patient follow-up.
- UCCP communication:
- Helping with UCCP registration.
- Pathology documentation/reporting.
- Reconciliation of invoices.

- Ability to fully utilize Electronic Medical Record (EMR) System.
- Excellent written and verbal communication skills.
- Ability to demonstrate office management practices, procedures, and use of equipment.
- 2-4 years medical office/healthcare environment experience preferred.
- Must be fluent in English and Spanish.

Salary : $25
