As a nurse at Phoenix, you get to help fulfill this mission! Many of our individuals have higher medical needs. As the nurse, part of your position is to help manage these needs while the individuals are experiencing life to the fullest extent possible.
A LPN or RN consultant nurse will have a caseload of 4-5 group homes. They would be expected to manage the medical needs of the clients at these homes and well as help educate the staff at these homes. This position requires flexibility in hours and days of the week to fit needs of the house as well as your needs. Position includes Monday through Friday on call and some weekends as well. Cooperative, positive and supportive communication skills are needed. This position is hourly.
LOCATION : Multiple sites in the Twin Cities area.
Duties : Must physically visit each home once a week. Some duties may be done remotely. Maintain regulation compliance. Evaluates conditions, activity ,care and problem solves, seeking assistance as needed. Does medical monitoring, assessments, and observations. Must maintain a positive working relationship with families, case managers, and coworkers.
QUALIFICATIONS :
- A current LPN or RN license to practice nursing in the State of Minnesota.
- Ability to organize and execute tasks independently.
- BLS CPR and first aid or the ability to obtain.
- Ability to chart in and navigate different computer programs.
- Certified Tardive Dyskinesia rater or ability to obtain.
- Experience in the treatment of individuals with developmental disabilities.
- Staff development training experience.
- Ability to assist with lifts and transfers
- Ability to sit, stand, push / pull wheelchairs.
- Ability to read, write and speak the English language.
- Ability to hear and see to make clinical assessments.
- Ability to travel between sites using own vehicle ; holds a valid driver's license and maintains a satisfactory driving record.
