Summary Description:
Assist agency RN/Healthcare Supervisor & Director of Nursing & Healthcare Services (DON/HCS) in overseeing health and safety supports for all individuals. Assist in completing all necessary medical documentation.
Duties and Responsibilities:
- Provide healthcare services for all program recipients
- Assist with all medical aspects of a program recipient’s admission process.
- Assist with coordinating medical services with each recipient’s primary physician when needed.
- Participate as a member of an inter-disciplinary evaluation team.
- Participates in ongoing interdisciplinary evaluations and review of each recipient’s needs and status (as assigned).
- Assist HCS with maintaining currency of records on an ongoing basis.
- Maintain medical records, health plans, medical recommendations/authorizations and medication information for each program recipient as it pertains to the day habilitation service plans.
- Obtain reports and approval of treatment plans from primary physicians; and ensure that records/documentation meets regulatory requirements. Meet or exceed regulatory requirements for CARF and Day Habilitation programming.
- Assist with updating monthly progress notes and semi-annual reviews.
- Assist with completing annual nursing assessments for recipients of Day Habilitation Service Plan.
- Assist with providing training and education to staff and individuals in relevant health care areas, including Safety, Universal Precautions, and others as requested by the Program Director.
- Participate in recipients’ day habilitation and residential service planning and other team meetings, as requested.
- Effectively communicate with state agency personnel, other service professionals, family members, guardians, etc.
- Provide on-site assessment and treatment of individuals in the day programs and in the residential homes, as needed.
- Administer medications and provide medical interventions as directed by physician order.
- Follow all regulatory requirements re: medication administration, security and storage.
- Provide assessment of individuals for self-medication (as needed).
- Provide consultation/staff training as requested by HCS.
- Assist program staff in providing direct care/personal care supports to individuals.
- Responsible for the administration of residential individual's medications that cannot be administered by MAP staff.
- Support program participants’ right to express their religious affiliations and cultural traditions through preferred and relevant activities. Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.
- Perform other duties as assigned.
