Supported Community Living Staff work within the community assisting our members with daily life skills.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Works directly with members
- Coordinates members transfers and/or discharges
- Maintains member case file records
- Prepares individual member plan semi-annually
- Prepares quarterly progress reports
- Documents members progress
- Serves as an advocate for the members
- Helps to secure appropriate transportation for individuals if needed
**Valid driver's license is required**
Job Type: Full-time
Pay: From $13.50 per hour
Expected hours: 40 per week
