Supported Community Living (SCL) is an important role in our organization and vital to our members independence. This position will travel within the Sioux City area to the member's home and assist them in their daily/weekly life.
Responsibilities/Duties:
- Works directly with members as directed in their case plan
- Coordinates member transfers and/or discharges as needed
- Social Activities within the community by taking members to functions.
- Establishes and maintains member case file records in a complete and accurate manner
- Prepares individual member plan semi-annually for each assigned program participant and attends meeting with members and other support persons
- Prepares quarterly progress reports for each assigned program participant
- Works with other SCL staff to insure program unity, communication and continuity
- Documents members progress on a regular basis in journals
- Documents members hours each month on a B-34 and submits them to the Administrator
- Is available to member in a crisis situation
- Serves as an advocate for the member
- Helps to secure appropriate transportation for individuals as needed or transports if necessary
- Must be at least 18 years old
- High School diploma
- Medication Aide or Medication Manager status
- Detailed oriented and highly organized is a priority
- Maintain professional continuing education
- Desire to work with persons with disabilities
- Valid Chauffeurs license or obtain within 6 months of employment
- Schedule will be Monday - Friday however employee will be on call every 3rd weekend
Pre employment physical and drug screen required. Clean driving record is essential to this position.
