What are the responsibilities and job description for the Tracker position at The Primary Health Network?
Position Overview
The Tracker is accountable for tracking patient referrals and testing, requesting correspondence for completed referrals or tests, performing necessary outreach to encourage patient compliance, and completion of fully tracked orders. This position will work collaboratively with site care coordinators or Centralized Care Coordinators, providers, clinical staff and outside facilities to ensure timely tracking and completion of open orders to prevent gaps in care. The use of computerized databases and electronic records will facilitate the required tracking.
Location and Hours of Position
The position is located in Sharon, Pa.
The general office hours for this position are 8:30 AM-5:00 PM. M-F
NO WEEKENDS
Compensation
Position starts at $16.00 per hour, however, PHN recognizes experience, education and site location when determining wage.
Education/Experience Requirements
High School Diploma or equivalent
2 years clinical experience preferred.
