What are the responsibilities and job description for the CASE MANAGER position at The Salvation Army Great Lakes Division?
Provide case management services for the clients of The Salvation Army Harbor Light.
Essential Responsibilities :
Conducts didactics / educational lectures
- Maintain case records in accordance with Harbor Light standards
- Serve as Case Manager to treatment clients
- Assists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)
- Participates in the review of all clinical issues, activities and program policies and procedures
- Attends all clinical staff meetings, and team conferences
- Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills
Qualifications :
Ability to determine needs and to work with multi-problem individuals
Education / Experience :
A bachelor's degree in social work or an equivalent comparable human services field is required
Computer Skills :
Excellent computer skills required
Certificates and Licenses :
Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
Physical Demands :
Ability to lift 10 pounds
In addition : Must be highly organized, motivated, a self starter with the ability to multi-task, be creative.
