Shift: Sunday-Thursday 3pm-11pm
POSITION TITLE: Medication Assistant
SUMMARY: Provide personal care services, assistance, support, and supervision to residents. Administer routine medications to resident in accordance with state regulations.
REPORT TO: WELLNESS DIRECTOR
ESSENTIAL JOB: Follow the assistance plan in providing services to residents.
RESPONSIBILITIES:
1. Assist in the development and updating of each resident’s.
assistance plan.
2. Dispense all prescribed medication as ordered by the physician.
3. Maintain an accurate Medication Administration Record.
4. Maintain and count an accurate Controlled Substance Record.
5. Inform Administration of any change in a resident’s condition.
6. Follow up with pharmacy to ensure in proper and expedited delivery of all medication
7. Encourage socialization among residents.
8. Encourage and/or remind residents to participate in activities.
9.Promote activities enthusiastically to pique residents' interest.
10. Chaperone activities outside the facility when requested.
11. Monitor the health, safety, and well-being of the residents.
12. Document information pertaining to changes in residents'
functional status, personal needs, appointments, etc.
13. Assist in keeping the environment safe for residents.
14. Provide one-on-one support/visitation to assigned residents on a
Weekly basis or as requested by The Wellness Director or as specified in the
Assistance plan or daily communication log.
15. Report concerns or observations about residents well-being to The Case Manager or Administration.
16. Respond to resident emergencies; provide first-aid assistance and
Arrange for appropriate medical attention/follow-up.
17. Arrange for transportation services for transfers.
18. Identify supplies needed to perform tasks and report needed supplies to The Wellness Director.
19. Practice safety in working around and with equipment,
chemicals, tools, and utensils.
20. Carry out other duties as assigned
