SPARC Services and Programs is seeking multiple Qualified Professionals (QPs) to be Care Managers. We have Care Manager positions open in the Greater Charlotte, Greater Greensboro, and Greater Gastonia/Shelby area. SPARC is dedicated to the mission of Keeping People out of Institutionalized Care we provides behavioral health services to children, families, and adults across the Piedmont and Mountain Regions of NC.
We are looking for a dedicated Tailored Care Managers for our TCM Program.
Qualifications & Experience:
- A minimum of a bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN); and
- Two years of experience working directly with individuals with behavioral health conditions.
- Preferred experience includes working with adults and children with MH and/or SUD needs
Primary Responsibilities-
The primary responsibilities of the care managers are to provide whole person integrated care management services to members who are enrolled in the Tailored Plans.
Provision of Care Management Services-
The following activities will be completed by the Care Managers via in person and telephonic support of their clients:
- Care Management Comprehensive Assessment
- Care Plans
- Development of the Care Team
- Care Coordination
- 24/7 On Call Crisis Support (on call coverage is rotational)
- Transitional Care Management
- Health Promotion
- Medication Monitoring
- Address Unmet Health Related Resource Needs
- Promote System of Care
