What are the responsibilities and job description for the Assistant Coordin of Adult Day Services position at The Summit Center?
ESSENTIAL DUTIES AND RESPONSIBILITIES: The Assistant Coordinator of Community Services must be able to consistently and successfully perform all essential functions of the position.
Rate of pay: $21.00 - $23.50/hour
Position is Monday through Friday 8am-4pm
- Ensure that individuals are offered personal choice and are supported in making their own decisions to the greatest extent possible.
- Ensure that individuals participate in their community to the greatest extent possible in the most integrated setting appropriate to their needs.
- Ensure the safety of all individuals during the provision of services.
- Maintain the rights of individuals being served.
- Assist in administering all processes relating to assigned programs.
- Work cooperatively with the individuals and the individuals' support teams to identify areas of need and individuals' valued outcomes.
- Complete and maintains accurate documentation relating to the various services provided to individuals and their progress toward achieving their identified valued outcomes.
- Assist with managing funding and departmental expenses for assigned Community Services programs.
- Assists in hiring, training and supervising staff and volunteers in the implementation of best practice models to meet identified habilitative goals. Assists in training and supervising staff in accordance with the DSP workforce transformation competencies and agency requirements.
- Ensure regular communication with individuals (and/or their families and/or group home staff) enrolled in programs.
- Ensure timely and accurate completion of incident, injury reports or other reports.
- Safely transports individuals to/from various community sites, activities, and home.
- Share in 24-hour emergency on-call phone rotation as needed.

- Perform any other duties as requested by your supervisor.
- EDUCATION and/or EXPERIENCE: B.S. Degree in Human Services field AND two(2) years of experience in the field of developmental disabilities or High School Diploma and 2 years' experience in the field of developmental disabilities. One year of Supervisory experience required.
