Duties:
- Participate in touring and orientation of potential residents/family members.
- Conduct outreach and education on facility services and availability of patient care services to professional contacts, such as physicians, hospitals, related medical professionals, and discharge planners.
- Welcoming and facilitating new admits into the skilled nursing setting, with education and follow up as necessary.
- Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed and necessary for patient pre and post admission needs.
- Assist family and resident as necessary in the execution of the required admissions documents.
- Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. Provide direct assistance and support to residents/families with the process of application, submission, and coordination with the relevant programs and their offices.
Requirements:
- High school diploma
- Experience in skilled nursing environment, or related health field, as well as experience in public outreach and education. On-the-job training provided for specific tasks.
- Supervisory and/or administrative experience in rehabilitative long term care preferred.
- Knowledge of the Medicare PPS system preferred.

Pay: $50,000.00 - $65,000.00 per year
