The Ambulatory Social Worker has responsibility to provide care/service safely and efficiently for a full range of services to patients of all ages and their families in both clinic and home health and hospice settings. Primary role is to collaborate, communicate and facilitate coordination of services. Responsible for the psychosocial assessment of patients, coordination of care and linkage to community resources and providing emotional support to patients and their families. Ensure appropriate decision makers are informed of and involved in treatment planning. Apply pertinent state and federal regulations regarding documentation and reporting requirements. Responsibilities and Essential Job Functions- Complete psychosocial assessment on patients/family/support situations including social, psychological, emotional, financial, and other relevant factors to facilitate patients’ linkage to resources to support care in the community.
- Identifies and utilizes all relevant information (medical/nursing needs, social work knowledge base, disease process, knowledge of community resources) to accurately and thoroughly assess the patient’s psychosocial situation.
- Documents appropriate information in the patient’s medical record to ensure communication of patients’ psychosocial needs for care in the community. Documentation includes initial contact and follow-up action plan.
- Initiate timely plan for plan of care and modify plan as needs are identified and resources are determined.
- Participate in multidisciplinary health care team meetings, provide relevant information in relationship to patient, options, and services. Identify actual and potential challenges and facilitates interventions to mitigate barriers to successful implementation of care plans.
- Maintain regular contact with patients, families, and caregivers regarding anticipated needs, plans, date, treatment options, resources, and assistance, etc.
- Advocate on behalf of patients, families and caregivers’ access to services; assist in navigating complex social systems.
- Maintain current knowledge of available resources and maintains adequate supply of resource materials for patients, families, and caregivers.
- Educate patients, families and caregivers on effective ways to mobilize existing resources.
- Complete reports to Protection Report Center on suspected patient abuse, neglect, and or exploitation.
- Demonstrate flexibility and teamwork, assist staff in the event of fluctuating census, patient emergencies, etc.
- Plan daily routine to properly organize tasks and demonstrate willingness to meet immediate patient needs effectively and efficiently.
- Supports the financial viability of the health system utilizing established procedure and appropriate resources to expedite safe and timely discharges.
- Works in partnership with physician and nursing staff to ensure timely linkage to services.
- Utilizes established procedures and appropriate resources in working with third party payors to ensure safe and timely coordination of care.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Masters Social Work
Preferred Education and Experience
- 1 or more years experience in case management/care coordination/discharge planning.
- Experience in health care setting
Required Licensure and Certification
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) or much complete BLS certification within the first two weeks of employment. Licensed Masters Social Worker(LMSW) - State Board of Behavioral Sciences must be obtained within 60 days of hire
