Job Summary:
Works under the supervision of the RN case manager or the Social Worker. The position is responsible for supporting the coordination of care activities. May interact with insurance companies, home care agencies, skilled nursing facilities, durable medical equipment companies, or transport agencies.
Education & Experience
High school diploma or GED required. Health care experience preferred.

Knowledge, Skills & Abilities
Demonstrated ability to perform the essential functions as outlined. Demonstrated human relations and effective communication skills. Demonstrated knowledge of medical procedures and terminology. Ability to handle highly sensitive or confidential issues. Ability to successfully manage multiple priorities. Ability to work independently and as a team player.
Responsibilities:
- Implements patient discharge plans prepared and delegated by social work or nurse case management team members. This is done by coordination with home care agencies, post-acute care facilities, durable medical equipment companies, transportation agencies and others as indicated.
- Assists in collecting data related to patient outcomes and auditing data for accuracy.
- Documents activities within appropriate medical record, or in other ways as directed.
- Issues CMS forms (Important Message, MOONS, etc.) to patients/designee.
- Ensures referral information is accurate.
- Follow up on delays in patient transition including, contacting facility to confirm referral was received; assure all appropriate documents have been sent; contact facilities daily until patient transferred; arrange appropriate transportation.
