Job Summary:
Enhances the patient experience by providing a seamless navigation process from diagnosis through end of care. Responsible for collaborating with dental and dental hygiene students, faculty providers and members of the multidisciplinary teams to triage, coordinate, and consistently manage patient care by assuming the role of resource person for the patient and the multidisciplinary team. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.
Education & Experience
Education and Experience Required:
Associate’s Degree or completion of Dental Assistant Program and three (3) years experience in a clinical dental healthcare environment.

Knowledge, Skills & Abilities
Knowledge, Skills, and Abilities:
Knowledge of dental office procedures, protocols, and operations. Verbal and written communication skills. Ability to train and motivate others.
Responsibilities:
- Interacts regularly or as needed with the multidisciplinary team, clinic staff, clinical support services, social workers, dietary, patient resource services, financial counseling and other hospital services to ensure integrated care.
- Navigates patient through continuum of care
- Serves as liaison for patients/families, providers, nursing and other appropriate staff
- Works with support staff to coordinate and schedule patients appropriately
- Proactively assesses the needs of the patient prior to arrival through review of clinic provider treatment and appointment schedules
- Provides a visible and specific channel through which patients can seek solutions to problems concerns, and unmet needs
- Develops, improves, and coordinates a system which organizes the flow of patients from referral through treatment, including evaluation and education and support
- Educates patients (and possibly family members) on topics such as the nature and status of patient's
- Participates in and/or leads quality improvement processes to enhance the operations of the program and overall patient satisfaction.
