Job Summary:
Collaborates with members of the multidisciplinary treatment team to assist patients and their families with adjustments to emotional, physical and cognitive issues that are affecting adaptive functional ability or resulting in lifestyle changes. The Social Worker plans, implements, and evaluate evidenced-based interventions through application of critical thinking skills and synthesis of information, while coordinating discharge planning and care management activities, as well as addressing the psychosocial implications of healthcare resulting in and from acute and chronic illnesses/injuries.
Education & Experience
Graduate from an accredited social work program with a Master's of Social Work and licensed in the state of Mississippi as a Licensed Master Social Worker(LMSW)

CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED: MISSISSIPPI LICENSURE (LMSW) REQUIRED.
Knowledge, Skills & Abilities
Clinical work with diverse populations and groups; formulating biopsychosocial assessments; Understands the forms and mechanisms of oppression and discrimination with the ability to apply strategies of advocacy and social change that advance social and economic justice; Be able to use theoretical frameworks supported by empirical evidence to understand individuals' development and behaviors across the lifespan and the interactions among individuals, and between individuals, families, groups, organizations, and communities; critical thinking and communication skills to evaluate and form professional judgments in practice; serves as Social Work Expert surrounding medical and behavioral health social work needs;

RESPONSIBILITIES:
  • Assessments: Completes assessment and provides crisis intervention for patients and family members presenting to the Emergency Room for care. Conducts, gathers and documents detailed psychosocial and/or biopsychosocial assessments/evaluations to identify social, environmental, and medical needs of the patients and families at all stages of development, as well as develop intervention and treatment strategies. Meets the needs of transplant applicants and donors by preparing reports and recommendations to the multidisciplinary treatment team; Completes Pre-admission Screenings (PAS) for skilled nursing facilities; Assess patients to determine the need for Level II Preadmission Screen/Resident Review.
  • Treatment and goal planning: Implements interventions and treatment plans that promote patient and family well-being and ensure continuum of care. Provides or supervises plan of care interventions to include resource and referral assistance, prevention, education, patient advocacy, bereavement, therapy, and other assistance as appropriate.
  • Discharge Planning: Arranges referrals to appropriate community services and resources/agencies while coordinating the post discharge continuum of care; Identifies high risk populations and promotes coordination of care to prevent readmission; ensures patient has a safe discharge plan in place prior to discharge; arranges for transition of patient to alternative levels of care if unable to discharge to previous living situation ton include referrals for mental health and chemical dependency treatment for children, adolescents and adults.
  • Psychosocial Intervention: Identifies high-risk social situations to intervene and coordinates appropriate interventions and referrals in cases involving the disabled, substance use/abuse, suspected or actual cases of child and vulnerable adult abuse, neglect or exploitation, as well as domestic violence; Complies with mandatory reporting requirements for cases of abuse, neglect and violence while collaborating with local, state, federal agencies/officials, as well as school districts and other community entities as assigned or prescribed.
  • Crisis Intervention: Assists patients and families in crisis situations to restore equilibrium to their biopsychosocial functioning while minimizing the potential for psychological trauma as a result of crisis occurring at the personal or societal level; provides brief treatment and counseling to patients and families dealing with crisis situations and issues of death and dying.
  • Financial Resource Management: Acts as a liaison to ensure uninsured or underinsured clients are connected with appropriate health benefits programs to secure financial resources, as well as assists with completion of respective program applications as needed;
  • Advocacy and Education: Serves as an educator for patients, families, the community and members of the multidisciplinary treatment team regarding disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens. Identifies quality and/or risk management issues and assures appropriate notification/referral. Serves as a cultural liaison to interview and gather vital information about a patient's culture, norms, background, and beliefs to ensure optimal care of the patient.
  • Performance Improvement & Continuing Education: Participates in ongoing evaluation of social work and coordinated care practices to assess quality and appropriateness of services, to improve practice, and to ensure competence. Assumes responsibility for continuing education and professional development in accordance with National Association of Social Work at the state and national levels.
  • Clarification of legal and ethical issues: Provides counseling and education on advance directives as needed. Assist in establishing clarity of kinship, legal next of kin, guardianship and/or conservatorship. Identifies ethical issues and dilemmas while utilizing ethical principles to contend with the demands of reconciling conflicts
  • Field Instruction and supervision: Serves as a field instructor for social work students at baccalaureate and/or master's levels; provides supervision to LMSW's in pursuit of the LCSW licensure.

Requires occasional exposure to unpleasant or disagreeable physical environment such as high noise level and exposure to heat and cold, occasional handling or working with potentially dangerous equipment, occasional exposure to biohazardous conditions such as risk of radiation exposure, blood borne pathogens, fumes or airborne particles, and/or toxic or caustic chemicals which mandate attention to safety considerations, occasional working hours significantly beyond regularly scheduled hours, occasional travelling to offsite locations, occasional activities subject to significant volume changes of a seasonal/clinical nature, occasional work produced subject to precise measures of quantity and quality, frequent bending, occasional lifting and carrying up to 75 pounds, frequent climbing, frequent crouching/stooping, occasional driving, occasional kneeling, frequent pushing/pulling, frequent reaching, frequent sitting, frequent standing, frequent twisting, and frequent walking.
