About This Opportunity: The University of Vermont Health Network is seeking an experienced Registered Nurse to join our team as a Clinical Care Specialist. In this role, you will be responsible for evaluating patient eligibility for our services and supporting patients, their families, community members, and referring providers in understanding the breadth and depth of our services.
This is a unique opportunity to make a difference in the lives of our patients and their families. You will work collaboratively with our team to ensure that we have all required information and documentation for a complete referral, and coordinate with referring providers and internal directors and managers to support patient access to our services.
Requirements:
- 3 years worked experience as a Registered Nurse
- Live within driving distance to the Colchester, VT office or willingness to relocate
- Prior home health experience preferred
