What are the responsibilities and job description for the PHSO Resource Coordinator- Hybrid/Remote position at The University of Vermont Health Network?
The Population Health Services Organization (PHSO) has a full-time opening for Resource Coordinator. This is a hybrid/remote role that has occasional travel onsite to outpatient clinics in Addison, Washington, and/or Chittenden counties.
JOB DESCRIPTION:
Coordinates the care and services of selected patient populations across the continuum of illness, with a focus on addressing the Social Determinants of Health; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payors and the community.
EDUCATION:
Associates Degree in Human Services. Bachelor’s level degree in Human Services preferred. Candidates with high school diploma and relevant work experience in a healthcare setting may be considered.
EXPERIENCE:
2-3 years of experience in a healthcare setting (or equivalent)
Care Coordination experience highly preferred. Customer Service experience preferred
