Job Title: Rehabilitation Services Coordinator

We are seeking a skilled Rehabilitation Services Coordinator to join our team at The University of Vermont Health Network. In this critical role, you will be responsible for coordinating rehabilitation services across various departments and ensuring seamless patient care delivery.

Responsibilities:

• Coordinate referrals and transfers between rehabilitation services and other healthcare teams;

• Manage patient flow and scheduling to optimize rehabilitation delivery;
\
• Communicate effectively with patients, families, and healthcare teams to ensure accurate information sharing;
\
• Facilitate efficient communication among rehabilitation teams to resolve any issues or concerns;
\
• Maintain accurate records and reports on patient progress and department functions;
\
• Collaborate with healthcare teams to identify opportunities for process improvements;
\
• Ensure compliance with relevant policies and procedures.

Requirements and Qualifications

• Bachelor's degree in Healthcare Administration, Physical Therapy, or related field;
\
• At least 1 year of experience in healthcare coordination or rehabilitation services;
\
• Excellent communication and interpersonal skills;
\
• Able to multitask and manage competing priorities effectively;
\
• Strong analytical and problem-solving skills;
\
• Proficiency in electronic medical records systems.
