Clinician, Adult Services : Half time in our Adult Outpatient Clinic providing individual, group and case management services to adults. Half time providing services off site for Acute Community Support Team (ACST) is a higher level of care intended to treat, stabilize, and support clients with Severe and Persistent Mental / Emotional Illness whose symptoms tend to be barriers and interfere with access to treatment. We are based off SAMHSA's ACT services. Clients may have co-morbidity with that of Substance Use. Its overarching main goal is to support clients access to treatment, which means we may meet in the community (in-home, in-shelter), help clients attend appointments through arranging transportation, and when it makes sense to, to bring them to appointments. We work to stabilize with the hope of eventual return to outpatient care after stabilizing, as well as help them build a better quality of life through access to care.

Clinician, Adult Services : Half time in our Adult Outpatient Clinic providing individual, group and case management services to adults. Half time providing services off site at a contracted location. The clinician in this role will work 20 hours off site with 16-to-24-year-old females who have experienced extensive trauma. Some clients may be involved in the criminal justice system or other state agencies, some may have no formal or sustained employment history and are likely young parents. The main goal in this role is to support clients access to treatment, which may involve meeting in the community (in-home, in-shelter) to build trust and rapport and work with clients on their behavioral health needs. Care coordination and collaboration with program staff is a key component of the position to engage these youth in services and supports.

KEY RESPONSIBILITIES

  • Direct services may include, but not limited to :
  • Conducts (comprehensive) clinical assessments and engagement

  • Determines DSM V Diagnosis in consultation with supervisor, Medical Director, Psychiatrist, medical provider or designee as needed.
  • Develops treatment plans and contracts for services
  • Implements treatment plans, modifying it as appropriate during the course of treatment.
  • Discharge planning, which may include linkages to community support services
  • Provides case management services
  • Completes clinical documentation required by agency and program policies, HIPAA regulations, and funding sources
  • Maintains productivity targets determined by service
  • Participates and presents in Peer Consultation and Quality Assurance Reviews.
  • Fosters collaboration with other clinical and administrative staff to respond to client and programmatic needs, including the implementation of agency policies, procedures, and initiatives.
  • Partners with other program or clinical staff to identify and develop approaches to address clients' unmet needs or service gaps.
  • Acquires and maintains the needed skills in evidenced based models that the agency / program ascribe to.
  • Partners with Supervisor to identify and develop plans for professional growth.
  • Contributes to program functioning by assuming responsibility for special tasks or leadership in special projects as appropriate.
  • Contributes to agency organizational life by participating in agency wide task forces, committees or problem-solving teams as appropriate.
  • Contributes to agency visibility and development of community linkages by representing the Village in community groups or committees.
  • Experience and Skills

    EDUCATION / QUALIFICATIONS

  • Master's degree in social work, psychology, or other licensed counseling degree.
  • State of CT license required (i.e. LCSW, LPC, LMFT, Psychologist)
  • Minimum of 2 years of experience postgraduate in an outpatient setting (depending on program
  • Bilingual / bicultural skills preferred.
  • Cultural competency to serve a diverse population.
  • Strong diagnostic, assessment and treatment skills for individual, family and group therapy.
  • Extensive experience working with culturally diverse populations, a wide age range, and a variety of clinical presentations.
  • Knowledge of behavioral health, child welfare systems, and community resources.
  • Good organizational and communication skills.
  • Ability to function autonomously in day-to-day responsibilities; yet seek consultation appropriately when needed.
  • Proficiency with word processing and general computer skills.
