Responsibilities:
Promote caregiver and patient support and education through the provision of dementia support groups, community resources and individual visits.
- In coordination with the Dementia Care Specialist, provide high quality programming to new and existing patients and families living with dementia.
- Ensure high quality Brain Gym program delivery and individual interaction with patients with dementia.
- Participate in family conferences alongside Dementia Care Specialist and primary care medical teams, as appropriate.
- Maintain confidentiality.
- Update Dementia Care Specialist with patient progress notes.
- Act as a promoter and resource to patients, providers, staff and community on health education and wellness topics and services.
- Other duties as assigned.
Education/Experience Requirements:
- No education requirement.
- Experience working with individuals with dementia and their families preferred but will train.
- Ability to develop and co-facilitate hour-long physical and mental Brain Gym program for patients with dementia.
- Must be proficient in Microsoft Office and have the ability to quickly learn any specific departmental software that may be required.
- Must be extremely organized and flexible to adjust to multiple task requests and changing priorities.
- Proven experience as a self-starter.
- Recognized experience in dementia education.
- Excellent organizational and time management skills
- Outstanding verbal and written communication skills
- High level of emotional intelligence
- Setting, defining, assigning, monitoring and evaluating outcomes of tasks and goals.
- Skill in problem solving
- Ability to read, interpret, and apply policies and procedures.
- Ability to identify problems and recommend solutions.
