Key Responsibilities :
- Perform comprehensive medical assessments and ongoing evaluations for clients in rehab and sober living.
- Diagnose and treat acute and chronic medical conditions , including those related to substance use.
- Provide medication management , including MAT (Suboxone, Vivitrol, etc.), in compliance with state and federal regulations.
- Monitor clients for withdrawal symptoms, medication side effects, and overall health status .
- Collaborate with psychiatrists, therapists, case managers, and other healthcare providers to develop integrated treatment plans .
- Educate clients on disease prevention, wellness strategies, and harm reduction approaches .
- Provide referrals for specialized care when necessary.
- Maintain accurate and up-to-date electronic health records (EHRs) in compliance with HIPAA and facility policies.
- Participate in staff meetings, training sessions, and case consultations to ensure coordinated care.
- Ensure compliance with Kentucky state medical regulations and licensing requirements .
- Active and unrestricted FNP license in Kentucky.
- Board certification as a Family Nurse Practitioner (FNP-BC or FNP-C).
- DEA license (or eligibility to obtain) for prescribing medications, including MAT.
- Experience treating substance use disorders, co-occurring mental health conditions, and chronic diseases .
- Knowledge of medication-assisted treatment (MAT) and withdrawal management.
- Strong understanding of trauma-informed care and harm reduction principles .
- Excellent communication, documentation, and collaboration skills .
- Ability to work in a fast-paced, multidisciplinary environment .
- CPR / BLS certification (or willingness to obtain).
- Experience working in rehab, sober living, or community health settings .
- Familiarity with Kentucky Medicaid, insurance billing, and documentation requirements .
- Training in motivational interviewing, SBIRT, or other evidence-based approaches for addiction recovery.
- Experience with electronic health record (EHR) systems .
