Job Purpose: Provides case management and supportive services reflective of the Wellspring mission that are designed to strengthen the well-being and development of families and individuals.
Experience and Knowledge
- Bachelor’s degree and minimum of 2 years work or related field experience working with homeless population and Veterans
- Knowledge of community resources
- Previous experience working with the public

Principle Duties and Responsibilities
- Determines nature of client’s situation by:
- Assessing participant’s needs and determining the nature of the situation through screening and interview process.
- Establishes course of action and assistance for client by:
- Completing the diversion process by locating safe, appropriate alternatives to shelter when applicable.
- Ensures efficient flow of services for clients, visitors and callers by coordinating the Intake process, assessing needs and determining priorities.
- Assessing immediate needs of individual client situation through screening process
- Educating participants on services and participant rights
- Assist persons served in resolving and mitigating barriers
- Establishing linkages with appropriate agencies and service providers for appropriate referrals
- Advocating on behalf of client when action is needed
- Fosters client’s action or adjustment by:
- Interpreting attitudes and patterns of behavior.
- Reviews and highlights new options.
- Empathetic listening.
- Maintains record of case by:
- Documenting client’s situation and client’s own actions.
- Documenting advocacy for client.
- Assuring that records are sent to Outcomes Monitor in timely manner.
- Monitors planned actions by:
- Periodic follow-up.
- Maintains operations by:
- Following policies and procedures.
- Participating in quality reviews.
- Completing paper work in a timely manner.
- Following safety procedures at all times.
- Maintains client confidence and protects operations by:
