Position Title: Community Health Advocate I (Community Health) 

Department: Healthy Communities 

Reports to: Program Manager, Community Health 

FLSA Classification: Non-Exempt 

Compensation: $25.00 - $27.00/hourly 

Schedule: Part-Time; 20 hours per week 

 

Background: 

In the midst of the Black Power and Civil Rights movements, four Black mothers saw a need in West Oakland and delivered. From their love and commitment Baywell Health started out as The West Oakland Health Council. We quickly became the neighborhood’s anchor for healthcare, health education, food and housing programs, and voter registration.

Job Summary: 

Under the general supervision of the Community Health Program Manager, the Community Health Advocate I works to conduct outreach that identifies individuals with unmet social support needs, provides community resource and referral assistance to patients and community members, and advocates for the coordination of healthcare and social support services for those individuals.  

 

Duties/Responsibilities: 

  • Conduct community outreach to identify students with unmet needs. 

  • Build the patient population by engaging with the student body. 

  • Assist students and community members with Medi-Cal enrollment on campuses and at community-based events. 

  • Use screening tools to identify students' social determinants of health needs (e.g., homelessness, food insecurity, low literacy, exposure to violence). 

  • Coordinate health services, assist with referrals, and ensure ongoing care. 

  • Conduct classroom presentations, workshops, health fairs, and other events. 

  • Educate on topics such as pregnancy prevention, HIV/STI prevention, substance abuse prevention, and violence prevention. 

  • Provide counseling on healthy behaviors and offer general support to patients. 

  • Promote healthy lifestyles and behaviors through education on chronic condition management, student engagement, and health access retention efforts. 

  • Actively engage students in health programs and request feedback for improvement. 

  • Attend meetings with campus organizations and officials to coordinate health services and programming. 

  • Shadow senior staff for program implementation. 

  • Participate in trainings and care coordination meetings. 

  • Assist with other Community Health programs and provide backup for CHA or Baywell Health team members. 

  • Performs other related duties as assigned. 

  • May be assigned cross departmental responsibilities. 

 

Required Skills/Abilities: 

  • Skills in promoting health equity through motivational training, programs, and policies. 

  • Bi-lingual competency highly desired. 

  • Understanding of health equity, health education, and unmet social determinants of health. 

  • Knowledge of healthy lifestyle practices, nutrition, and fitness. 

  • Proficiency in basic computer functions and tools such as Excel, Word, and other health data management tools. 

  • Excellent verbal and written communication skills.  

  • Excellent interpersonal and customer service skills. 

  • Excellent organizational skills and attention to detail. 

  • Excellent time management skills with a proven ability to meet deadlines. 

  • Ability to function well in a high-paced and at times stressful environment. 

  • Must possess a valid driver’s license. 

 

Education and Experience: 

  • High school diploma or equivalent required. 

  • 1-2 years of experience in a community health setting or a combination of education and equivalent experience. 

  • Experience working with underserved or minority populations is a plus. 

 

Mental and Physical Requirements:  

  • The pace of Work: High stress, very rapid, and demanding 

  • Concentration: Constant 

  • Standing and Walking Time Required: At least 20% of the time 

  • Sitting and typing: 60% - 70% 

  • Environmental and Health Hazards: Frequent exposure to communicable diseases and other conditions common to a clinic environment. 

  • The employee must frequently use hands and fingers to operate a computer keyboard, mouse, and telephone keyboard and talk or hear. 

  • The employee must occasionally bend, lift and move up to 25 pounds. 

  • Specific vision abilities required by this job include close vision, peripheral vision, and the ability to adjust focus. 

  • May be required to work in an environment with high noise levels and unpredictable temperature and ventilation.
