Case Planner
MINIMUM QUALIFICATIONS
- Bachelor's Degree, preferably in Social Work, Human Services, or a counseling-related field, and direct experience working with youth in a counseling position (copy required upon hire)
- Valid New York State Driver’s License (copy required upon hire)
- Yearly Physical (required within one month after the hire)
- Ability to work with a diverse population presenting a variety of cultural, socioeconomic, developmental, and sexual identities. Must have the ability and willingness to establish and foster healthy and supportive relationships that acknowledge individual identities and encourage personal growth.
PREFERRED QUALIFICATIONS
- Master's Degree in Social Work and previous experience in working with critically disturbed adolescents

HOURS
- 40 hours per week

SALARY
Starting at $54,075 annually
POSITION OBJECTIVE
Under the supervision of Behavioral Health Clinicians, provide effective clinical Case Planning services to youth with the goal of family reunification and positive reintegration into their home communities.
DUTIES AND RESPONSIBILITIES
- Responsible for the development and coordination of individual treatment planning for each resident and family. Implementation of the treatment plan and discharge planning along with appropriate continuing care recommendations.
- Maintain up-to-date and accurate information on each resident in the statewide Connections system, including progress notes related to their duties, incidents of family contact, resident contact individuals, and group counseling, as required by the New York State Office of Children and Family Services.
- Coordinate appropriate family/resource visitation in conjunction with the treatment team.
- Provide ongoing updates and attend weekly supervision with the appropriate Behavioral Health Clinician.
- Coordinate clinical reports for legal hearings and document any legal activity in Connections with the Behavioral Health Clinician.
- Co-Chair treatment team meetings in conjunction with the Behavioral Health Clinician.
- Attend all meetings and training as requested by the Director of Residential Services, Residential Department Head, Behavioral Health Clinician, and Human Resources Department.
- Perform other duties as assigned by the Residential Department Head.
- Adhere to the treatment philosophy of The William George Agency and the rules, requirements, and regulations of the New York State Office of Children and Family Services.
- Assist the Admissions Department with the intake process.
- Responsible for adhering to and performing all duties in accordance with every safety standard, practice, and policy of The William George Agency. This includes but is not limited to, a review of The William George Agency’s Safety Policy and attending all required safety-related training.

The William George Agency for Children’s Services is located on a 650-acre campus nine miles northeast of Ithaca, in the Finger Lakes Region of New York State. It is a private not-for-profit residential treatment center licensed by the New York State Office of Children and Family Services (OCFS) that serves both male and female youth between the ages of 12 and 21 and their families who present with significant mental health issues, behavioral difficulties, and substance abuse issues. Our programs offer an array of treatment modalities specifically designed to meet the needs of our youth.
