Family Services Coordinator

MINIMUM QUALIFICATIONS :

  • Master's Degree, preferably in Social Work, Human Services, or Counseling, or Bachelor's Degree and 3-5 years of direct experience working with youth and families in a counseling position (copy required upon initial hire date), with a commitment to obtain Master’s degree within five (5) years
  • Valid NYS Drivers License (copy required)
  • Yearly Physical (required within 1 month of initial hire date)
  • Ability to work with a diverse population presenting a variety of cultural, socioeconomic, developmental, and sexual identities. Must have the ability and willingness to establish and foster healthy and supportive relationships that acknowledge individual identities and encourage personal growth.

  • SALARY :

    70,000-90,000 / Annually

    POSITION OBJECTIVE :

    To effectively work with families in order to implement the treatment planning process with families and or permanency resources to facilitate the goal of family reunification and to work with the family to develop and implement a continuing care plan and establish effective permanency.

    DUTIES AND RESPONSIBILITIES :

  • Locate, identify, and establish appropriate primary and alternative discharge resources for residents, in conjunction with their Permanency Planning Goal, as defined by the Family Court System.
  • Work closely with the Behavioral Health Clinician to stay informed about residents’ progress in treatment and effectively communicate progress to families.
  • Provide family support services and casework to the families as indicated in the treatment plans, as well as keep families updated about their youngsters’ progress in treatment. There should be bi-weekly face to face contact with families whose youngster is currently in placement.
  • On an ongoing basis, provide information about the Agency programs and admission criteria and procedures to OCFS, Probation Departments, Family Courts, School Districts, and other human service agencies.
  • Provide liaison services to the referral agencies and other human services agencies in the community in order to facilitate the progress of children and their families.
  • Provide after-care services to children and their families for six (6)months after discharge from the Agency. As needed by a particular county.
  • Provide support services to the families, i.e., help gather information about community resources, and help obtain financial, transportation, food, and shelter support.
  • Be an integral part of the child’s treatment team by providing input to the team about family issues, dynamics, and needs by participating actively in the formulation of treatment plans. Whenever possible, the Family Services Coordinator should attend the Comprehensive and Treatment Plan Reviews.
  • The Family Services Coordinator should meet at least monthly with their Residential Department Head for supervision, and weekly with the Behavioral Health Clinician for supervision and guidance.
  • Help develop discharge plans. As needed, make referrals for aftercare services, including but not limited to services through the Office for People with Developmental Disabilities, Mental Health, etc. Assist families in identifying and securing aftercare resources.
  • Represent the Agency at Family Court hearings and present the Agency’s assessment and recommendation regarding the children’s and families’ treatment needs.
  • Perform Agency public relations functions as directed by the Residential Department Head.
  • Perform other duties as assigned by the Director of Admissions and the Residential Department Head.
  • Responsible for adhering to and performing all duties in accordance with every safety standard, practice, and policy of The William George Agency. This includes but is not limited to, a review of The William George Agency’s Safety Policy and attending all required safety-related training.
  • Adhere to all rules, regulations, policies, and philosophy of The William George Agency and the New York State Office of Children and Family Services.

    POSITION LEVEL : Responsible to the Behavioral Health Clinician and Residential Department Head (Director of Developmental Achievement Services, Strive Services, or Special Services) for all treatment and liaison functions

    The William George Agency for Children’s Services is located on a 650-acre campus nine miles northeast of Ithaca, in the Finger Lakes Region of New York State. It is a private not-for-profit residential treatment center licensed by the New York State Office of Children and Family Services (OCFS) that serves both male and female youth between the ages of 12 and 21, and their families, who present with significant mental health issues, behavioral difficulties, and substance abuse issues. Our programs offer an array of treatment modalities specifically designed to meet the needs of our youth.
