The Wright Center Medical Group is seeking a highly skilled Addiction and Co-occurring Disorders Service Navigator to join our team. In this role, you will be responsible for providing comprehensive case management services to individuals with addiction and co-occurring disorders.
The ideal candidate will have a strong understanding of behavioral health issues and the ability to work effectively with diverse populations. They will work closely with our interdisciplinary team to develop and implement personalized service plans that address the unique needs of each client.
Responsibilities include:
- Actively participating in client and program-related trainings
- Providing for continuity of effective client and family care by acting as a liaison/communication link between teams
- Assisting program staff in data collection and fulfilling grant requirements
- Maintaining an information/referral list of community services
