Summary :
The Congestive Heart Failure (CHF) Program Specialist assists in ensuring a quality-based and patient-focused program that is devoted to appropriate management of the population with congestive heart failure. Implements key protocols and processes for managing referrals, communicating patient information, developing and setting goals with the patient, scheduling appropriate evaluations and follow-ups, and managing the database to track relevant patient information and activity. Coordinates patient flow from presentation clinically to the heart team review, to scheduled visits, to clinical testing and evaluation, and procedure setup. Key Accountabilities:
Collects comprehensive data pertinent to the patient’s health and situation. Identifies and reaches out to patients with complex care needs. Establishes relationship with patients and assesses the patient’s subjective experience. Assessment is dynamic and on-going. Helps client assess readiness and available resources for change. Acts as clinical resource and provides clinical assessment and decision-making as needed during shift. Conducts on-going identification of psychosocial barriers to receiving care.
Analyzes the assessment data to determine patient nursing diagnosis and issues. Together with client, explores assessment data to determine areas for change.
Identifies expected outcomes for a plan that is individualized for a patient or a situation.
Develops and implements a plan that prescribes strategies and alternatives to attain the expected outcomes that incorporate evidence-based practices. Together with client, develops a coaching plan that identifies strategies to attain goals. Adjusts plan as desired by the client. Utilizes a variety of specific coaching and communication skills to facilitate learning and growth.
Coordinates care delivery with multidisciplinary team members and others. Delegates tasks and other duties appropriately providing adequate supervision
Evaluates progress towards attainment of expected outcomes. Partners with the client to evaluate progress toward attainment of goals. Arranges and tracks patient referrals to system and community resources
Collaborates with patients, family, health care providers and others in the conduct of nursing practice. Develops positive working relationship with patient, primary care provider, and other essential care team members. Collaborates with others to assist clients in achieving goals. Participates in care team meetings providing updates of patient’s health care goals, activities, and action plan. Defines processes for most effective patient flow.
Performs Disease Registry management.
Qualifications:
Bachelor’s degree in nursing
Minimum of one year of nursing experience
CPR and ACLS
Current State of Wisconsin registered nurse license
Physical Demands:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of fifty (50) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Work Environment:
Frequent exposure to sharp objects and instruments
Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock
Occasional high noise level in work environment
Possible exposure to communicable diseases, hazardous materials, and pharmacological agents
Occasional contact with aggressive and or combative patients
Position requires compliance with department specific competencies.
