Job Overview
We are seeking a dedicated and detail-oriented Intake Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing patient intake processes, ensuring that all necessary documentation is completed accurately and efficiently. This position requires strong communication skills, a solid understanding of medical terminology, and proficiency in various electronic health record systems. The Intake Coordinator will be responsible for facilitating patient service and coordinating care plans to enhance overall patient experience.
Responsibilities
Placement Coordination:
a. Complete and coordinate placements for Austin and San Antonio.
b. Gather or send needed documents at placement
c. Assist with in person placements, as needed.
Statewide Intake Coordination:
a. Complete intakes Statewide for non-emergency placements in a timely manner.
Facilitate Education and Court Liaison for Austin and San Antonio:
a. Ensure that the transfer of the most recent educational records and material is obtained for TFL records, via the school’s electronic system.
b. Troubleshoot any issues with school enrollment and alternative programs (ex; ECI, HeadStart, ect.) and assist as needed.
c. Evaluate the immediate needs of children to determine appropriate referrals and next steps at placement.
d. Assist with attending court hearings, as needed.
Documentation and Record-Keeping:
a. Communicate all documentation and information regarding new children with appropriate case manager within 14 days of placement.
b. Assist with gathering of information to be utilized as screening criteria for the agency Intake Committee (composed of Program Coordinator, Intake Case Manager, Executive Director, and Director of Quality Assurance)
c. Communicate with other Child Placing Agencies and referring agencies to collect all necessary documentation to accompany a child entering TFL
d. Follow up to initial contact and upload all necessary documentation that would accompany a child transferring from another agency or documentation that would accompany a child entering foster care for the first time.
e. Maintain accurate and up-to-date records of placement activities, including documentation of placement decisions, family assessments, and follow-up reports.
f. Ensure that all required documentation is completed and filed in accordance with agency policies and regulatory requirements.
Any other duties requested by supervisor.
Requirements
- Bachelor’s degree or equivalent with experience at higher level administrative position. At least one year experience working in either a child placing agency, related mental health field, or children’s services.
- Previous experience in social services, child welfare, or a related field is preferred. Experience in a placement or case management role is highly desirable.
- Training in the development of treatment plans for clients.
- Experience with STAR Health coordination and educational advocacy is highly desirable.
- Proficiency in Microsoft Office and other relevant software applications.
- Proficiency in Teams.
