Primary Purpose and Function
Responsible for coordinating the aftercare planning for all IOP, PHP, & RTF patients. Works with THIRA’s patient care team to determine the best coordination of care. Initiates and executes all outside referrals and communications.
Required Qualifications:
· Bachelor’s degree or four (4) years relevant and progressively responsible experience; or equivalent combination of education and experience
· Minimum two (2) years of healthcare administration experience
· Demonstrated competency in the use of Microsoft Office and the ability to use various electronic software programs
· Experience with Electronic Medical Records (EMR) systems and other medical practice software
· Must be able to successfully pass a comprehensive criminal background check (Federal and state)
· Ability to work evenings, nights, weekends & holidays as needed
· Perform all other duties as assigned
Preferred qualifications:
· Prior experience with behavioral and mental health patient discharges
· Prior experience with Kipu and Salesforce
· Prior experience with healthcare marketing and business development
· Experience working in PHP, IOP, or IPU environment(s)

· Research and maintain current knowledge of community resources and understanding of referral partners; ensure accuracy of provider contact information, specialties, insurance, and availability
· Initiate set up of aftercare plan; liaise with patient-facing team to support with referral information as needed
· Assist in contacting providers, scheduling pertinent assessments and appointments as it relates to patient’s discharge plan
· Identify patient barriers to care, such as transportation, language, insurance, etc.; coordinate with patient-facing team to refer patients as needed to additional resources
· Attend clinical huddles, present patient information and share progress updates
· Meet with Residential and PHP patients each week of program, as determined by SOPs
· Provide discharge paperwork to patients on their last day of treatment; follow up with patients electronically as needed; ensure outpatient providers receive records following a patient’s discharge from THIRA; Provide resources for unplanned discharges
· Conduct post-discharge check-in with patients, as determined by SOPs
· Document all discharge planning communications; discharge patients from EMR system, Salesforce, and update relevant trackers
· Support and train new Admissions & Discharge team members
· Support Admission & Discharge Manager as needed

Work Environment
Work is performed primarily indoors. Lighting is artificial with some natural. Equipment used includes standard office equipment, i.e. computer, telephone, fax machine, copier, printer, and scanner. Physical demands include sitting, walking, standing, bending over, repetitive use of hands, and lifting up to 25lbs. Position requires close contact with all THIRA Health staff and mental health patients. Hours and days can vary within a Saturday through Friday work week.
