Starting Pay: $16.55 per hour for this entry-level role

Schedule: Monday through Friday, flexible hours


Currently, we are seeking a highly organized and detail-oriented individual to join our team as a Community Psychiatric Support Treatment Case Manager. As a CPST Case Manager, you will provide care coordination, case management, behavioral health treatment intervention, and community support services to clients and families to establish and maintain client’s stable functioning in the home, community and school settings. Engage in comprehensive, on-going assessments of client’s ecology to determine family and system identified needs, strengths, resources, stressors, and sources of support to be used in treatment planning, providing Community Psychiatric Supportive Treatment (CPST) and Therapeutic Behavioral Service (TBS).


Job Duties:

  • Provides care coordination, case management and mental health intervention to Severe & Persistent Mental Illness clients providing Community Psychiatric Support Treatment (CPST).
  • Assess and identify clients' medically necessary needs, empowering the client and family by enhancing their ability to identify areas of need and seek out appropriate resources with the goal of greater independence.
  • Develop and implement treatment plan goals and achieve stated outcomes.
  • Utilize a Wraparound approach in service delivery and promote alignment of practice with wraparound values.
  • Provide evaluation services, education, intervention, and aftercare to individuals and families experiencing mental health and substance use concerns. Provide behavioral health interventions to clients; train and empower parent/guardian to provide similar interventions in the home when applicable.
  • Develop and maintain cooperative working relationships with other persons and/or groups providing services to clients experiencing substance use disorders and their families, e.g. Alcoholics Anonymous, Alanon, inpatient/residential programs, court and/or probation officers, etc.
  • Responsible for the vital interests of the client and family by maintaining a working knowledge of ecological/community resources. Possess knowledge of services available to client's experiencing substance use disorders and their families, and the methods of obtaining these services.
  • Work collaboratively with all team members, including agency and center staff, parents, and other professionals to implement the program and achieve the best possible outcomes for every client.
  • Assist with the assessment, management, and resolution of crisis situations with clients and their families which require 24/7 on call responsibilities periodically.
  • Provide hospital liaison contact/consultation in the event that a client is hospitalized including discharge planning.
  • Demonstrates competency in current treatments, especially those deemed to be Best Practices.
  • Exhibits ability to de-escalate client crisis situation in an appropriate manner.
  • Communicates needed information respectfully and clearly with clients, providers, and staff.
  • Ensure confidentiality practices are always maintained. Abide by and have knowledge of agency confidentiality policies including HIPAA policies and procedures.
  • Maintain client records and charts in a timely, confidential and complete manner.

Requirements

Required Qualifications:

  • High School Diploma or its equivalent required.
  • Ohio CDCA Certification, preferred.
  • Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation.
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen
  • Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation/credentialing.
