Thom Child & Family Services, located in Lowell, MA is seeking a Community Health Worker to join their team. This is a flexible schedule up to 25 hours per week Monday through Friday.
Qualifications: The Community Health Worker is a Public Health outreach, resource and referral specialist who can apply a unique understanding of the experience, language and/or culture of the communities served by the Early Intervention Partnerships Program.
Objective: Help referred and program eligible individuals/families to make connections with appropriate health care, human services, and other community resources. Driving between community agencies to support families with needed resources. Mediate, when necessary, between families/individuals and community health or human services system. Participate in direct services with Partnerships Nurse(s) and Mental Health Clinician(s) as needed, providing assistance to individuals/families through interpreting, informal counseling and social support.
Position Responsibilities:
- Help referred and program eligible individuals/families to make connections with appropriate health care, human services and other community resources by:
- Making referrals to other services including helping individuals/families to meet their basic needs.
- Motivating individuals to seek care:
- Remind individuals of appointments
- Assist in scheduling CHA (Community Health Agency) visits with appropriate EI Partnerships staff
- Provide individuals with resources regarding transportation and providing follow-up contact as necessary
- Mediate, when necessary, between families/individuals and community health or human services system by:
- Translating literal and medical languages
- Collecting information from clients that may be inaccessible to other health and human service providers
- Helping to build individual and community capacity by bringing about community participation in health activities
- Provide assistance to clients re: accessing insurance and community resources
- Assist, as appropriate, in submission and tracking EI Partnerships data.[PW4]
- Participate in documentation when visiting with clients and other administrative duties as required
- Participate in direct services with Partnerships Nurse(s) and Mental Health Clinician(s) as needed by:
- Providing assistance to individuals/families through interpreting
- Providing direct services and assistance to individuals/families through informal counseling and social support including facilitating support groups
Position Requirements: The applicant must demonstrate:
- Knowledge and understanding of the issues facing Early Intervention Partnerships individuals/families
- Knowledge of the community and local resources.
- Ability to form trusting, non-judgmental, and supportive relationships with new parents, their children, and other family members
- Respect for diverse family structures, practices and beliefs, particularly related to health and parenting
- Interest, ability and dedication to working as part of an interdisciplinary team
- Bilingual candidate preferred (Spanish, Haitian Creole, Portuguese)
- High School Diploma or GED required
- Valid Driver License/Reliable Means of Transportation Required
Supervision: The Family Liaison reports to and is supervised by the Early Intervention Partnerships Program Coordinator. Face-to-face supervision occurs (at minimum) once every two weeks for up to one hour or as needed. Supervision may be either individual or as part of a group and may be both clinical and administrative in nature.
