Job Description
The advanced practice provider, either a Nurse Practitioner or Physician Assistant, works in collaboration with and is an integral member of a multidisciplinary healthcare team within the acute care hospital setting. This role provides comprehensive care on the inpatient Hospital Medicine service.

The advanced practice provider, either a Nurse Practitioner or Physician Assistant, works in collaboration with and is an integral member of a multidisciplinary healthcare team within the acute care hospital setting. This role provides comprehensive care on the inpatient Hospital Medicine service.
Key Responsibilities:
- Perform complete history & physicals and document them in the medical record.
- Order and collect data using appropriate assessment techniques, relevant supporting diagnostic information, and diagnostic procedures where indicated.
- Serve as the first responder for both nursing and house staff in situations requiring procedural and resuscitative interventions.
- Assess risks associated with the care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
Diagnosis and Management:
- Collaborate with the interdisciplinary healthcare team to make diagnoses of acute and complex chronic conditions.
- Manage diagnostic tests through ordering, interpretation, performance, and supervision.
- Formulate differential diagnoses by priority.
- Diagnose complications and order appropriate interventions.
Plan of Care:
- Identify expected outcomes from diagnosis and formulate a plan of care to address complex acute and chronic healthcare needs.
- Utilize evidence-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
- Implement and modify the plan of care.
- Presume diagnostic strategies and therapeutic interventions.
- May perform advanced procedures consistent with privileges and competency validation.
Communication and Collaboration:
- Maintain ongoing communication and collaboration with the interdisciplinary healthcare team.
- Facilitate and communicate with patients, families, and staff to promote continuity of care across the inpatient and outpatient continuum of care.
Documentation:
- Document key components of patient progress via daily progress notes, transfer summaries, discharge summaries, and clinic notes where applicable.
- Documentation must be timely, meet acute care compliance standards, and capture patient acuity.
Professional Practice:
- Demonstrate Professional Practice behaviors, including preceptorship/mentoring, education, and instruction of graduate students.
- Seek opportunities for active engagement in research and analysis of evidence-based practice.
- Actively participate in Advance Practice Grand Rounds and specialty-specific meetings.
- Maintain CEUs and membership in a professional organization.
