Overview The PATH Coordinator will play a key role in supporting individuals with intellectual and developmental disabilities (IDD) as they transition from institutional settings at Rainier School to independent community living. This part-time role involves on-site support, coordination with Rainier staff, and community outreach to ensure a smooth and successful transition process for each mentee.
Key Responsibilities
1. Program Coordination (3-4 Days Per Week)
- Coordinate and manage PATH program activities focused on individualized support and goal tracking.
- Conduct regular site visits at Rainier School and other community settings.
- Build and maintain relationships with Rainier School staff, community partners, families, and guardians to create a strong support network for participants.
2. Mentor Recruitment and Development
- Recruit, train, and provide ongoing supervision to Peer Mentors.
- Implement a mentorship framework that includes ASL training and other communication skills to foster inclusivity.
- Conduct regular check-ins and provide feedback to Peer Mentors to ensure high standards of support.
3. Transition Planning and Engagement
- Design and implement tailored transition plans for mentees, addressing their unique needs and goals.
- Coordinate community integration activities, events, and social opportunities to build skills and confidence in mentees.
- Work with families and guardians to keep them informed and engaged throughout the transition process.
4. Administrative and Reporting Duties
- Maintain accurate and detailed records of all program activities and communications.
- Prepare regular reports, summarizing program progress, challenges, and successes.
- Manage scheduling, logistics, and other administrative tasks to ensure seamless operation of the PATH program.
Compensation and Schedule
- Starting Wage: $30.00 per hour, with an increase to $32.50 per hour after a 90-day probation period.
- Schedule: Part-time, 3-4 days per week, with flexibility based on site visit needs and program demands.
Qualifications
- Experience in IDD Services: Minimum of 2 years working directly with individuals with intellectual and developmental disabilities (IDD), with demonstrated knowledge of support needs, transition processes, and advocacy for individuals with IDD.
- Leadership Skills: Experience in a supervisory or mentorship role, ideally involving recruitment, training, and development of peer mentors or similar roles.
- Organizational Skills: Demonstrated ability to manage schedules, coordinate multiple weekly site visits, and communicate effectively with diverse stakeholders (e.g., guardians, staff, mentees, families).
- Strong Communication Abilities: Proficient in both verbal and written communication, with the ability to convey program information and engage effectively with mentees, families, and partner staff.
- Valid Driver’s License and Reliable Transportation: Essential for making regular site visits as part of the position.
- Technology Proficiency: Comfortable using Google Workspace, MS Office, and program-related databases to document visits, progress notes, and communications.
Preferred Qualifications
- Educational Background: Bachelor’s degree in social work, psychology, education, human services, or a related field, or equivalent experience.
- Experience in Transition Planning: Previous experience developing and implementing transition or support plans for individuals moving from institutional to community settings.
- Knowledge of ASL or Other Communication Supports: Familiarity with American Sign Language (ASL) or other communication supports for non-verbal individuals is an asset.
- Experience with IDD Community Engagement: Background in coordinating community events, family engagement activities, or outreach events that foster community connections for individuals with IDD.
- Problem-Solving Skills: Proven ability to address challenges creatively and develop solutions for mentee engagement, communication barriers, and other potential program challenges.
- Flexibility and Adaptability: Able to work effectively in a flexible, part-time position with a variable schedule and adapt to program needs as they arise.
