Overview:
We are seeking a compassionate and organized Care Coordinator to join our team. The Care Coordinator will play a crucial role in ensuring the well-being of individuals in need of care services. This position offers the opportunity to make a meaningful impact on the lives of those requiring assistance.
Duties:
- Check in medications and facilitate medications that come on cycle
- Working with RN and Admin to get medication orders from doctors offices
- Monitoring medication passing
- Communicating with Home health and Hospice companies about care and medications for Residents
- Creating care plans
- Communication to families about the Residents needs and any cares that change
- Helping to train new CNAs
- Helping with tours if needed
Skills:
- Experience in caregiving or related field such as assisted living, home care, or long-term care
- Strong understanding of behavior management techniques
- Excellent communication and interpersonal skills
- Ability to work effectively and be a self motivator
