Travel RN Case Manager – Gainesville, FL & Tallahassee, FL
Locations:
Gainesville, FL 32614 – Must have Meditech, Midas, and Careport experience
Tallahassee, FL 32310 – Must have HCA and Midas experience
Duration: 13 Weeks
Shift:
Gainesville: Monday – Friday, 8:00 AM – 4:30 PM (occasional rotating weekend)
Tallahassee: Monday – Friday, 8:00 AM – 5:00 PM
Pay Rate: $52/hr
Job Description:
Are you an experienced RN Case Manager looking for an exciting travel opportunity? Join our team at top-tier acute care hospitals in Gainesville and Tallahassee, FL, where you’ll play a key role in patient care coordination, discharge planning, and utilization review. Key Responsibilities:
✔ Collaborate with interdisciplinary teams to develop and implement effective patient care plans
✔ Ensure smooth transitions of care by coordinating discharge planning and referrals
✔ Utilize Meditech, Midas, and Careport (Navihealth) for documentation and case tracking (Gainesville)
✔ Utilize HCA and Midas systems (Tallahassee)
✔ Conduct utilization reviews and ensure compliance with hospital policies and payer requirements
✔ Engage with patients and families to educate them on post-discharge care
✔ Work Monday-Friday with occasional rotating weekends (Gainesville only)
Requirements:
Active RN License (Florida or Compact State)
Minimum 2 years of hospital Case Management experience (Acute Care required)
Bedside patient care experience is a plus
Experience using Meditech, Midas, and Careport (Navihealth) – Gainesville
Experience using HCA and Midas – Tallahassee
ACM or CCM certification preferred but not required
Strong communication and organizational skills
