Position Summary:
At Thrive Behavioral Health Center, we deliver industry leading peer support for those suffering from Mental Health and/or Substance Use Disorders. A Certified Peer Supporter is a trained individual who has lived experience with addiction to alcohol and/or drugs who provides one-to-one strengths-based support to peers in recovery, empowering them to make healthier choices on a day to day basis. The Hospital & Community Linkage Peer Supporter will connect with individuals inside Emergency Departments immediately following an overdose. This program links survivors with recovery support services. The Certified Peer Support will create relationships with hospital staff, emergency room personnel, treatment centers, and local behavioral health / substance use disorder treatment agencies to create a coordinated response as an individual moves through appropriate levels of care. The Certified Peer Supporter will be an accountability partner to promote recovery by engaging in a personal and positive, supportive relationship, demonstrating and inspiring hope, trust, and a positive outlook and optimize client connection with a broad range of community based services and natural supports.
- Coordinate onsite visits to acute care settings, specifically hospital inpatient units and Emergency Departments
- Assist individuals on inpatient units after emergency department encounter navigate various levels of care as needed
- Assist peers in finding resources for harm reduction, detoxification treatment, family support, and support groups
- Work with community providers to ensure connection and adherence to to on-going care
- Work with individuals in the community once released from acute care settings to support transitional needs in the community and ensure connection to on-going care
- Responsible for maintaining accurate records and notes, confidentiality, and compiling timely reports
- Maintain records and provide assistance with appropriate documentation in files and computer database
- Provide recovery resources to individuals and their family members
- Advocate for the needs of each individual
- Create and maintain relationships with Hospital staff, Emergency Room personnel, mental health/ substance use treatment agencies and providers, and residential treatment providers
- Communicate with healthcare staff to build a rapport and coordinate care
- Other duties as assigned
Preferred Requirements:
- Prefer lived experience in an acute care setting; Identify as having received care in an Inpatient, Emergency room or residential setting.
- Experience with mental health preferred
- Certified Peer Supporter in the state of Ohio
Requirements:
- Identify as a person in recovery with at least one year of successful recovery
- Direct lived experience of mental health and/or substance use issues
- Reliable transportation
- Working knowledge of different pathways to recovery and social systems including evidence based practices
- Understands problems analysis, decision-analysis, evaluate interventions, and contingency plans
- Ability to be an active listener by asking questions and reflecting on what is being said
- Knowledge of area resources that may aid client in ongoing recovery (i.e. transportation, treatment providers, sober support systems)
- Proficient level of ability with computer; Microsoft Office Products
- Effective written and oral communication skills; ability to articulate information clearly
