Job Descriptions:
We are seeking a compassionate and skilled Hospice Social Worker, LMSW to join our interdisciplinary team. The Hospice Social Worker plays a crucial role in providing emotional, psychological, and social support to patients and their families as they navigate end-of-life care. This role involves assessing patients’ and families’ needs, connecting them with resources, and advocating for their well-being while respecting their values and wishes.
Key Responsibilities:
- Patient and Family Support:
- Conduct psychosocial assessments to evaluate the emotional, social, and financial needs of patients and families.
- Provide counseling to patients and families to help them cope with terminal illness, grief, and loss.
- Facilitate communication between patients, families, and the care team to ensure understanding and alignment with care plans.
- Care Coordination:
- Collaborate with the interdisciplinary team, including nurses, physicians, chaplains, and aides, to develop and implement comprehensive care plans.
- Assist patients and families in navigating healthcare systems, insurance, and end-of-life planning.
- Identify and connect patients and families with community resources, such as financial assistance, support groups, or legal aid.
- Advance Care Planning:
- Educate patients and families about advance directives, living wills, and healthcare power of attorney.
- Facilitate discussions about care preferences, goals, and end-of-life wishes.
- Crisis Intervention:
- Provide immediate support during crises, such as acute emotional distress or family conflicts.
- Help resolve issues that may impact the patient’s care or well-being.
- Documentation:
- Maintain accurate, timely, and confidential records of patient interactions and interventions in compliance with regulatory standards and organizational policies.
- Grief Support:
- Offer bereavement counseling and resources to families before and after a patient’s passing.
- Participate in bereavement programs and outreach efforts as needed.
Qualifications:
- Master’s degree in Social Work (MSW) from an accredited institution (required).
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), as applicable.
- Minimum of [1-2] years of experience in healthcare, hospice, or palliative care (preferred).
- Strong understanding of hospice philosophy, end-of-life care, and grief counseling.
